Contracting Questions
If you're an independent agent wishing to contract with NCD directly, reach out to Agent Support to get the application link.
If you're already working with an upline, you may need to reach out to them to get your unique contracting link. If they're not set up with us to bring on a downline, they can email Agent Support to make that happen.
In addition to the contracting link, you're going to need a few things:
- Your Individual NPN (even if you're applying as an agency!)
- Your current W-9
- E&O coverage documents
Then, simply send in the form! If Agent Support needs anything else from you, they'll reach out to you at the email you provided.
Welcome! Once your application has been sent in, our outstanding Agent Support team will complete your contracting audit. If they need anything from you, they'll email you directly.
As soon as your contracting audit has been completed, you should receive an email containing some key information, like your unique enrollment portal URLs and access to your back office.
We try our best to get contracting completed within 24 hours.
If we're missing something from you, or need to reach out, we'll contact you through the email or phone number you've provided to us.
Please contact Agent Support if you see this, or any other error, when trying to complete the enrollment application. We can check to see if you may already be in our system, or troubleshoot whatever issue is going on.

Yes! One contract with us is all you need.
When we complete your contracting audit, we ensure that your licenses are up to date in our system. This means that as soon as you receive your welcome email, you are ready to go in all of the states in which you currently hold active licenses.

Commissions Questions
For agents contracting directly with us, our street level commission rate is 21% for dental and 34% for vision.
For new business, commissions are paid weekly to agents.
For existing business, commissions are paid monthly to agents.
Agents continue to receive commissions for the lifetime of the policy.

Association Plans provide individuals access to the benefits typically available to larger organizations. By coming together and leveraging their collective bargaining power, association members gain access to more affordable, comprehensive coverage.
Associations provide members additional, exclusive benefits and resources, including access to their dental and vision plans. Association benefits include health, wellness, personal, and business benefits.
NCD by Met Life
All NCD by MetLife plans are paired with a membership to the National Fitness and Wellness Association (NWFA). The NWFA monthly membership fee varies depending on the dental plan selected, and is included in the members' monthly billing.
The NWFA provides non-insurance benefits that help members and their families and pets stay strong, healthy, and active. Click here to learn more about the NWFA.
VSP Vision
All VSP Preferred Vision plans are paired with a membership to the NWFA as well.
NCD by Nationwide
All NCD by Nationwid plans are paired with a membership to the National Small Business Association (NSBA). The NSBA monthly membership fee is $3.00 and is included in members' monthly billing.
The NSBA provides non-insurance benefits like discounts, rewards, and perks on thousands of brands in a variety of categories. Click here to learn more about the NSBA.
No. The membership to the association is what allows members to gain access to the rewards and benefits, including the dental or vision plan itself.
Currently, none of our dental plans cover orthodontics. However, our NCD by MetLife plans do include a discount to Smile Direct Club, which uses clear aligners to adjust tooth alignment issues.
All of our dental plans cover implants, but there are some specific guidelines about when and how those implants can be covered. For more clarity, reach out to your Account Executive, or to Agent Support.
Yes! Both the Nationwide and MetLife dental plans have a missing tooth clause. To get the specific details about what that means for your clients, reach out to your Account Executive, or to Agent Support.

For the 1500 plan, the price is $57 per member.
For the 3000 plan, the price is $72 per member.
For the 5000 plan, the price is $82 per member.
For the 1500, 3000, and 5000 plans, there is:
- A $50 annual deductible for Basic and Major services (per person)
- A maximum $150 deductible per family
For the 1500, 3000, and 5000 plans, there is:
- No waiting period for Preventive and Basic services.
- A 12-month waiting period for Major services.
No, we no longer accept proof of prior coverage as of January of 2023.
Yes, all NCD by Nationwide plans include a $3 NSBA membership fee.
Like many dental plans, NCD by Nationwide has some specific guidelines around dental implants. According to the Nationwide Dental Implant Rider,
Implants and Implant supported prosthesis covered under this plan are limited to;
- the replacement of permanent teeth extracted while insured under this plan, or
- the replacement of a prior Implant if it has been at least seven years since the prior insertion, and is not and cannot be made serviceable.
Implants will be covered under Procedure Class C subject to
- the Plan Year Benefit Maximum, and
- Percentage of Covered Services, and
- Deductible as shown in the Schedule of Benefits
Implants (so long as they meet the above criteria) are covered when placed for a removable denture. Implants are available to Covered Persons over the age of 16.
Submission of a Treatment Plan is required for implant benefits to be considered.
