Members Dental Benefits
Sign into your Member Portal to find out what plan you have and the services it covers. Through the Member Portal, you can verify benefits, download ID cards, review product information, update your payment method, and more!

You are able to view your remaining and expended benefits by visiting your Dental Claims Portal via https://main.mbaadmin.com/member/member_login . The Username is your Member ID number, and the password is your Date of Birth in long form (mm/dd/yyyy). Do not include the slashes when you are entering this information.
You can utilize our Dental Provider locator by clicking the Locate a Provider tool on the home page. By using this link, you can search by zip code for both general practitioners and specialists, or even search by a specific dentist’s name!
Upon enrollment, you should receive an email titled ‘Welcome Letter’, containing your log-in credentials. If you have access to your member ID number, you can update your username and password through the ‘Need to register’ button in the Member Portal
If you do not remember your log-in credentials and do not have access to your member ID or Welcome Letter, we can assist with changing your Username and Password over the phone! Upon enrollment, you should receive an email titled ‘Welcome Letter’, containing your member ID number. If you are unable to locate this information, ensure that you have checked your spam and junk-mail folders. It is also located on your member id card, which is mailed to you 10-14 business days after your effective date. You may send us a request for a new ID card through your Member Portal ! You will also find temporary ID cards in the portal, ready to be used while waiting for the physical copy to arrive!

In order to add a dependent to your plan, the insurance company requires proof of a Qualifying Life Event. These are any major life changing events that provide documentation, such as the loss of existing coverage, loss of coverage with employer, becoming a U.S. Citizen, Marriage Certificates, Birth Certificates, and more. As our company has grown, we have made changes to our plan identifiers and billing processors in order to increase our recognizability amongst our members. Whenever you see the ‘NCD’ tag, you can be confident that the charge is for your dental and/or vision plan(s).

When enrolling in a NCD dental plan, you automatically become a member of the National Small Business Association (NSBA). NSBA offers benefits such as medical bill negotiation, as well as discounted hearing products through EPIC Hearing Healthcare and discounted vision services through Outlook Vision. Your provider will most likely recognize the name of the network rather than the name of the plan. NCD is a PPO plan that utilizes the MaximumCare PPO networks, including Careington, Connection Dental, and DenteMax.
Members Dental Claims
When you use an in-network provider, the provider will file the claim for you. When you use an out-of-network provider, you may have to file the claim yourself. If you do need to submit a claim yourself, the ADA Claim Form is available to download through your Dental Claims Portal via https://main.mbaadmin.com/member/member_login . The Username is your Member ID number, and the password is your Date of Birth in long form (mm/dd/yyyy). Do not include the slashes when you are entering this information. The Claims Address for your plan is located on the back of your ID Card.
The claims payor for our NCD dental plans is Merchants Benefit Administration (MBA). For assistance with Dental Claims, please contact the claims payor, Merchants Benefit Administration at (480) 776-5041 8 a.m. – 5 p.m. PST/MST.
You can view claims submitted by your provider by logging into the Dental Claims Portal via https://main.mbaadmin.com/member/member_login . The claims portal will also allow you to see if you have met your deductible and how much is remaining of your annual maximum.
