Pre-certification is recommended for services expected to exceed $1000.00.
Filing a Claim

Claims Status
If you have already submitted the claim and need to check on the status, you can contact MBA directly by email or by phone at 480-776-5041 8 a.m. – 6 p.m. CST.
MBA Provider Portal
You can visit the claims payer website at mbaadmin.com and click on Provider Eligibility. Enter your Provider Tax ID number and the patient’s Member ID number. This will allow you to see benefit information, coverage details, confirm if the waiting period has been waived, and view maximum remaining amounts.
CPT Code & Claims Lookup*
You can utilize the CPT Code and Claims Lookup tools by visiting the claims payer website at mbaadmin.com and clicking on Provider Eligibility. Log in to the Provider Portal by entering your Provider Tax ID number and the patient’s Member ID number. You can utilize the CPT Code and Claims Lookup tools for assistance.
*In-Network providers have immediate access to these tools. Out-of-Network providers gain access after claim or pre-determination submission.
